How FUE Works, Stage By Stage

Follicular unit extraction breaks into three distinct stages, and understanding which one determines your result changes how you evaluate a clinic.

Extraction. Individual follicular units are removed from the donor zone with a cylindrical punch under one millimetre in diameter. Harvesting is deliberately spread thinly across a wide area rather than concentrated, so that remaining hair covers the gaps and density never visibly drops in one patch. Published ISHRS guidance cautions against removing more than twenty to thirty percent of units from a single zone in one session, because over-harvesting is the one complication that cannot be reversed.

Site creation. Tiny recipient incisions are made across the thinning area, each at a specific angle, direction and depth. This is the stage that determines the outcome. Hair leaves the scalp at a shallow slant rather than perpendicular, and every hair in a region flows the same way — get the angle wrong and no amount of density will make it sit correctly. It is the stage where a physician’s judgement most directly determines the result.

Placement. Units are sorted by how many hairs they carry and placed accordingly: single-hair units at the leading edge where a soft irregular transition is what reads as natural, denser two- and three-hair units behind them building volume. Grafts spend as little time out of the body as possible.

You are awake for all of it under local anaesthetic. The uncomfortable part is the first few minutes of numbing injections; after that you feel pressure and tugging rather than pain. Most people watch something or sleep.

Then the waiting, which is the genuinely hard part. The transplanted hairs shed between weeks two and four — normal, expected, and alarming to nearly everybody. Regrowth begins around months three to six and final density lands at twelve to eighteen months.
