FUE Versus Older Transplant Techniques

The reason hair transplants had a bad reputation for decades is that the old ones deserved it. Understanding what changed makes it easier to judge what you are being offered now.

Punch grafts, the 1970s and 80s. Large circular grafts of four millimetres or more, each carrying many hairs, placed in regular rows. The result was the plug look — visible tufts in a grid, with no gradation and no irregularity. Nothing about it resembled a natural hairline.

Follicular unit transplantation, the 1990s onward. The insight that hair grows in natural clusters of one to four, and that those units should be dissected and kept intact. A large aesthetic leap. But the harvest was a strip of scalp removed from the back and sutured closed, which leaves a permanent linear scar.

Follicular unit extraction. The same intact units, taken out individually with a punch under a millimetre across. No strip, no sutures, no linear scar — instead a scattering of tiny circular scars spread thinly across the donor zone, invisible at normal haircut lengths. This is what we perform.

What did not change is that technique alone does not produce a natural result. The tells of a bad transplant today are the same as they always were: a ruler-straight leading edge, multi-hair grafts at the very front, hair placed at the wrong angle, and a hairline set too low for a face that will keep ageing.

All four are design failures rather than technical ones, decided before any graft is placed. Which is why the useful question about a modern clinic is not which technique it uses — nearly all use FUE — but how much examination and physician judgement went into the design.
