Why Transplanted Hair Is Permanent And Your Other Hair Is Not

Transplanted follicles are permanent. Follicles taken from the safe donor zone are genetically insensitive to DHT, they keep that insensitivity after being moved, and they go on producing hair for decades. That property — donor dominance — is the entire biological basis of the procedure.

Everything else on your head is unchanged. Native follicles in and around the recipient area still carry the sensitivity they always did. Surgery does not alter their genetics and does not switch off the process. They keep miniaturising on their own schedule.

Put those two facts together and a specific failure mode appears. Grafts persist while the hair around them thins, which over years produces a visible island of denser transplanted hair with recession opening behind it. The transplant did not fail. The plan did.

Two things prevent it. The first is designing for the pattern you will have rather than the one you have now — which usually means a more conservative hairline than is technically achievable today, and deliberately reserving donor supply for later. The second is a long-term medical plan for the native hair.

This is also why some people have a second, smaller procedure years later. Not because the first one failed, but because new loss appeared elsewhere. A plan that kept donor supply in reserve is what makes that possible; one that spent everything on a low dense hairline at thirty is what makes it impossible.

The practical consequence at consultation: be suspicious of any conversation that treats surgery as the end of the problem. It is one half of a plan that has to work for thirty years.
