How Prescription Hair Loss Treatment Is Tailored

Most medical hair loss treatment rests on two mechanisms, but the plans built from them vary widely — because the variables are you, not the drug.

Blocking DHT. Finasteride is an oral 5-alpha-reductase inhibitor that lowers DHT, the hormone derivative driving pattern loss. It is the most established option for slowing progression in men. It also carries a recognised risk of sexual side effects in a minority of users, which is precisely why dose, suitability and monitoring are clinical decisions.

Extending the growth phase. Minoxidil works differently, prolonging anagen rather than blocking DHT. Topical or oral, it commonly causes an initial shedding phase that people mistake for the treatment failing, plus scalp irritation in some users.

What gets tailored: which mechanism or both, route of administration, concentration, and whether a compounded topical formulation makes more sense than a standard one. Age, medical history, other medication, plans around fertility, tolerance of side effects and how fast the loss is moving all shift the answer.

Timing is part of the plan too. If loss is progressing quickly, stabilising medically before considering surgery is usually the right sequence — sometimes for a year or more. Placing grafts into an actively receding pattern is how people end up with an island of transplanted hair.

And the part that catches people out: these treatments control an ongoing process rather than curing it. Stop, and the hair they were protecting resumes thinning. Any plan should be built on the assumption of continuation, or it is not really a plan.
