Can you still go bald after a hair transplant?
The transplanted hair stays, but yes — the native hair around it can keep thinning, which is why untreated pattern loss can leave a transplant looking stranded.
A transplant relocates DHT-resistant follicles. It does not alter the genetic sensitivity of the hair it was placed among, and it does not switch off the underlying process.
So the grafts persist while the surrounding native hair continues to miniaturise. Over years that can produce a visible island of denser transplanted hair with recession opening behind it — the specific failure mode that makes a transplant look obvious in a way it did not at month eighteen.
Two things prevent it: designing the hairline for the pattern you will have rather than the one you have now, and a long-term medical plan for the native hair. Neither is optional if you want the result to age well.
